Complete the General OP Referral Form to refer a child or family to Morrison for outpatient mental health services. To be completed by any Department of Human Services (DHS) branch, MH service provider, school counselor or primary care clinic.
Please fax completed forms to 503-872-0659 or email centralintake@morrisonkids.org. We will follow up with the caregiver. You will not receive confirmation of contact with caregiver unless requested.
To check on the status of a referral, contact us at 503-258-4381 or centralintake@morrisonkids.org. Please provide your name, the name and DOB of the child referred, the date of the referral, and your contact information for return contact.
